to obtain a
new patient
packet. Our
friendly
staff will
assist you
in making an
appointment
to see one
of our
pain
management
specialists.
For
your
convenience,
our new
patient
forms may be
completed
prior to
your first
appointment.
Once you
have filled
out the
forms, they
may be
mailed,
faxed, or
brought to
the office
the day of
your
appointment.
Out of
courtesy to
our
patients, we
do our best
to remain on
schedule
throughout
the day.
Filling out
the patient
forms prior
to your
arrival
allows you
more time
with the
physicians
and less
time in the
waiting
room. If you
do not have
the
information
completed
prior to
your
scheduled
appointment,
we will have
to
reschedule
your
appointment
and there
will be a
$35 fee. If
you have
difficulty
printing the
forms,
please
contact our
office so we
may mail or
fax you a
copy. Thank
you for
helping us
provide the
best and
most
efficient
treatment
possible.
If you have
any X-Rays,
MRI's or
Doctor's
reports,
please bring
them in with
you to your
appointment
along with
your current
insurance
cards.
